I killed him on my first try without even healing so I though it was a joke boss like pinwheel or the demon of song.

It's not. I'm not even sure he uses his crawl at all if you stay near, and as long as you stay underneath him while he breathes goop you'll be fine even if you're against the wall since it only flows uphill. For fuck sake, his arms are safe spots, and the only vulnerable parts, you can't fucking fail.
