Danganronpa isn't actiony but hits that crazy/LSD/psycho/violent/bloody theme I think you're looking for.
If you want to slash, shoot, and blow up a ton of shit I recommend Shadow Warrior. FPS with a katana. Feels old-school looks modern.
Bad Mojo is not exactly violent and it's very short but it's a very unique gross out experience

Anyways if you want some ACTUAL psycho shit there's always Spec Ops: The Line. The first FEAR game is great if you want a boost of adrenaline, play it on PC though. The Evil Within if you want more of a 'slowly going nuts' feel.

Harvester, it's a point'n'click adventure game but if you're looking for blood and gore it should definitely fill the bill.
Includes themes such as: cannibalism, necrophilia, pedophilia, nuclear war, holocaust, suicide, pyromania, incest, etc. it's just all around fucked up game, and really funny at the same time.
Chivalry: Medieval Warfare
I don't know if there's enough people playing Dark Souls 1 nowadays, but making a soul level one darkwraith and menacing the fuck out of new players is easily the most cathartic thing I've ever done in video games.
Although there may not be that much blood and gore involved, you can really feel the fear and desperation in people you invade, especially when you get creative with it.
