There are some differences, there are three series on the Beat em' Up part of Kunio-kun alone.
- The short lived one with the characters in moderate body proportions instead of the usual midget selves, those games were sort of "more serious" in story.
- The ones with the characters in the Edo period.
- The one people mostly know, the midgets making rivals for BARF.

The two Super Famicom beat 'em ups are solid and the fighting tournament game for the NES has a translated version under the name "Kunio & Riki" which is worth a play. If you don't care at all about story then play them all or just start from the Japanese 3DS games in order of release.
