There's a good chance the battery in the cartridge has long since died, so chances are if he does turn it off, it will in fact wipe his saves.

Also there's no fucking way it's been on for 20 years, nip power outages are common as fuck.....although I guess it's technically possible he's a series of generators he's kept fueled for the last 20 years.

Only in gold and silver do they run out super duper fast since not only do they power the clock for time, but the backup ram memory. I still have my batteries working in some of my old red/blue/yellow cartridges.

Heck, I still have NES batteries working (zelda 1 and 2), and super nintendo ones. Some of the batteries last way longer than what they were suppose to, I guess it depends upon the storage conditions.
