The "real" Earth exists in the Mario universe. Remember that he used to be a plumber from Brooklyn. It's implied that the Mushroom Kingdom is in another world, perhaps accessed via pipe. Since Mario games have also specified that he and Luigi are twins born in the Mushroom Kingdom, the probable truth is that his parents are from Italy, found a way (pipe?) to the Mushroom Kingdom, had children there, and then returned to the "real world" for a while, probably to either Italy or Brooklyn. Mario and Luigi lived their very earliest childhoods in the Mushroom Kingdom, but when their parents brought them to the real world as toddlers or infants (haven't played Partners in Time, but I've heard it shows that they lived in the Mushroom Kingdom as babies) they either lived in Italy long enough to pick up accents as young children then moved to Brooklyn until adulthood, or simply moved to Brooklyn until adulthood and picked up the accent from listening to their parents talk to them a lot in childhood.

In Super Mario Sunshine, he says "arrivederci" when he dies sometimes (Italian for "goodbye") and I personally think the Italian level names ("Delfino" Plaza, "Sirena" Beach) and the rather Italian looking Delfino Plaza itself (which has accordian background music obviously evocative of Italy) are intended as shout-outs to Mario's Italian heritage.
