Fishing Hamlet, death of Kos, the Hunter's Nightmare as divine punishment from Kos over the slaughter of the fishing hamlet and the deicide the Hunters committed, Moon Presence is stolen and 'raised' by the Healing Church, Moon Presence offers Gehrman a deal - either to help continue the hunt or bring the doll to life or something - this deal is very bad and Gehrman becomes a prisoner in the Hunter's Dream along with a doll who isn't really Maria, then you get a series of hunters who work under Gehrman's mentorship via the dream, then you get the Good Hunter, and then the final battle between Good Hunter and the Moon Presence, then the "Childhood's Beginning" where the Hunters and Yarnham have been freed from the dream of the hunt and a new chapter in the city's history begins under the 'care' of the Good Hunter aka Squid Kid.
If you don't accept all this, then there's a great amount of ambiguity regarding the Moon Presence's relevance to other Great Ones, as well as how Gehrman contacted it in the first place. With the theory, Gehrman doesn't need to contact a new great one, just contact the great one the church stole from the fishing hamlet.

From what I've understood, Maria was Gehrman's pupil.
The man probably was one of Byrgenwerth teachers, becoming a hunter after the first signs of the beastly scourge appeared and before Laurence departure from Master Willem.
As we know, Old Blood ministration was the cause of the beastly scourge, so after the founding of the Healing Church (responsible for Blood ministration) the Hunters of the Old Workshop (such as Maria or Gehrman) were forced to retire, because the Church wanted its members to enforce the hunt.
This is due to the need of diversified branches (Executioners, Black and White Doctors, Ya'har Ghul Kidnappers, Choir) for having total control over the hunt and Yarnham.
The Fishing Hamlet was ravaged by The Old Hunters, students of Byrgenwerth, in search for creatures akin to Great Ones.
The corpse of the white creature on the shore was probably experiment conducted by Byrgenwerth to turn a human into a 'faux' Great One (like Rom) capable of giving birth to a true infant without having a miscarriage.
It's not clear what Byrgenwerth did with the 'Sweet Child of Kos' but its phantom created the Nightmare and imprisoned the Blood Drunk Hunters.
The Orphan of Kos is a materialization of Gehrman's crimes, inhabited by the child of Kos' phantom.
This is why it uses a scythe-like weapon, screams like Gehrman (it's the same audio file, but distorted) and grows a cape in its second phase.
The connection between the two it's also shown by the fact that, after you kill the black spirit coming out the corpse, the Doll says that Gehrman is finally sleeping, after many restless nights.
Maria felt sorry for all of this, put a grave with a lumenwood to let the spirits rest in peace (you can see it outside the lighthouse hut) and waited in the Clocktower to protect the corpse on the shore and the village.

Think about it: Maria was guilty over her role in the patients suffering, so it wouldn't be consistent for her to doom them to an eternity of suffering in the Nightmare. then in the early 90s there was another series of OVAs called New Dominion and it's even better (and also a subtle prequel to GitS)
and around the same time Shirow started a new Dominion manga called Dominion Conflict but only had the time to publish the first story arc (No More Noise) before his apartment got destroyed by the Kobe quake which made him go insane and dstroyed his life's work.
Then in the early 2000s there was a really ugly CGI adaptation called Dominion Tank SWAT but nobody paid attention.
