Usually the highest difficulty I can select. I feel it keeps me interested in the game if its challenging. I am also one of those autists that will repeatedly bang my head against the wall. Until finally the wall breaks, and I beat the part im stuck on.

Normal only on the occasion that I know the game has shit scaling with difficulty.
Generally the second hardest difficulty or the hardest that can be selected before beating the game.

Depends on the game, usually hardest or second hardest if the game gets too broken and you have to abuse game mechanics in order to win.

Normal, because that is the difficulty the game is designed at. That, or the game developers are stupid.
