Usually the highest difficulty I can select. I feel it keeps me interested in the game if its challenging. I am also one of those autists that will repeatedly bang my head against the wall. Until finally the wall breaks, and I beat the part im stuck on.

Depends on the game, usually hardest or second hardest if the game gets too broken and you have to abuse game mechanics in order to win.
