The reality is game development is reaching a plateau where costs are beginning to meet or eclipse potential profits. If you spent $10 million and 3 years in 2008 to make a game, in 2016 that money and time frame will get you a lot less. This is because the most cost-intensive part of the game development (graphics) need to keep getting better or your customers will complain.
Thus, other things start falling through the cracks, like quality control, complexity, and so forth.
The solution is to stop demanding the BEST GRAPHICS EVER of these developers, but that's unlikely to happen any time soon because the franchises that sell the most, that get a release every year, rely on it (or at least rely on visuals improving).

AAA devs are often making games for 2-3 platforms. Games are very, very complex. It's not always just a matter of changing a handful of numbers and suddenly there's no lag and you're not falling through the floor anymore. Problems can sprout from literally anything, and when your game code is millions of lines long, that can mean weeks or months of debugging, recompiling, and testing.
And when you've already been at it for three years, sometimes you can't get it all done before the publisher's boot is on your throat demanding you release that game NOW or so help me the contract will be shredded and your entire team will be out on their asses with a black mark on their employment history.
There is finite time, manpower, and money that goes into projects. Are some mishandled? No doubt they are. But it's not just a matter of putting more effort into it. Sometimes there's just no more money to get it done.
