If you don't go out of your way to kill innocent people in free roam then most of Niko's kills are either self defense, drug dealers, or people who basically deserved it.
And his narrative fits fairly well if you basically ONLY kill storyline mission characters.
It's not like its much better with GTAV, just because you have Trevor who would technically 'make sense' doesn't mean you can't kill random people as Michael and Franklin and feel that so called "disconnect".

It's to do with the tone though. Playing through GTAIV the tone was sometimes really jarring when it came to what the game wanted you to take seriously and then you have characters like Roman and Brucie hamming it up the next minute.
At least in GTAV the tone was consistent. The missions were mostly a bit tongue in cheek and very silly by comparison which matched the stuff you were doing out of main missions and the personalities of the characters.

But yeah, there is a turning point for Niko's character. He does start off saying he wants nothing to do with killing, helps Roman beat up a couple of loan sharks, kills some street hustlers who came after him and Little Jacob, and eventually crescendo-ing into him killing Vlad. At this point he's gotten himself in deeper than he can, and the only way he can make money is doing jobs for the Russians. When Demitri double crosses him he doesn't have much left, being stuck in Bohan bleeding money with Roman. He eventually is forced to kill cops to steal money from the bank.
Does that mean he's a good person? No, of course not. Not entirely. That's the whole point of the game though. Does that mean he doesn't feel remorse for what he does? Obviously he does, it can be inferred very much from Niko's actions and words that the things he does aren't out of enjoyment. He is a very cynical person though, and this shows.
I understand how some can feel the disconnect, but I personally didn't. And as such Niko is probably one of my favorite protagonists, and GTAIV is personally my favorite GTA game. It's more than just Niko though, it's very much the city and the feel of it. I've never been immersed more in a game than I had in IV.
