What games have the same feel as a spaghetti western? While I like RDR, it doesn't feel stylized enough to fit what I'm looking for.

As a huge fan of spaghetti Westerns and Western themed games, RDRedemption is the closest any game ever felt to me.
It had the violence, the starkness, the grotesqueness, not to mention the straight forward lifted soundtrack.
Actually one of the only other games I felt remotely had that feeling was Strangers Wrath.

They were filmed in Italy/Spain because they were primarily produced by low budget Italian/Spanish companies, and it was of course cheaper to just stay where they to shoot than it was to fly to the US. There was no shortage of good stereotypical "Western" landscape in the US. The cinematographers just made due with what they had.
