Almost every Bloodborne character has a great design, but Doll is particularly impressive, because it's very restrained and realistic, while also being very characterful and endearing.
Most jap character designers would have gone over the top with ribbons and frills and oversized bows because "muh victorian gothic doll" and would have completely spoiled it.
Actual victorian fashion was plenty cute as it was, and acknowledging that fact is very commendable.
Also, the "sickly cute girl" archetype has been done to death and bastardized to the point where it's an empty gimmick, but by going back to the roots of gothic romance and creating a character who could conceivably fit in there, they managed to revitalize that archetype.
Which is pretty much an exploit.

Folding/unfolding equipment will always be cool so long as it's done right. I recently played through Dishonored and while not as cool as the helmet animation, your dagger unfolds when you equip it.
