The best defensive type is Steel/Fairy, which has nine resistance, two immunities, and only two weaknesses.
Theoretically I think Fighting/Ice would be the best offensive typing, its STAB is super effective against 408 Pokemon. However, Ghost/Fairy has the widest coverage with only two Pokemon resisting it.
Defensively, Ice/Rock is the worst typing. It has two 4x weaknesses and four 2x weaknesses, with only four resistances to show for it.
Theoretically, Normal/Poison is probably the worst type combination offensively. A whopping 123 Pokemon are immune to its STAB, and it only hits 97 Pokemon for super effective damage (all of them from Poison).
However, in the most specific of scenarios, Ice/Water with Thick Fat has the greatest resistance (taking 1/8th damage from Ice), and Grass/Bug with Dry Skin has the worst weakness (5x damage from Fire)

No weaknesses doesn't mean best combination, Dark and Ghost have complete ass for offensive moves so you're more or less relegated to a defensive role, and both the 'mons with that typing could stand to improve in that department. Plus, you don't resist jack shit outside of normal/fighting/poison, which isn't exactly impressive. Steel/Ghost is fucking ridiculous on the defensive side, since it has 3 fucking immunities, and resists almost everything that doesn't hit it for SE damage. Aegislash would've been unstoppable in Gen V. Bug/Steel and Grass/Steel are also better defensive picks, with immunities or resistances on at least half the types with only a 1 or 2 weaknesses a piece.
As for offensive typings, Ground/Dragon is part of the reason Garchomp got so fucking big in the first place since EQ hits 5 types for SE, including the only type that used to resist Dragon moves.
Basically, anything Steel is going to be close to top-tier as a defensive type unless you get shafted with a shitty second type like Rock (poor Aggron) and anything Dragon is going to make for a solid offensive combo just because high BP moves and few resisting types.
Worst type combo goes to either Grass/Psychic or Rock/Ice for having shit offenses and shit defenses all around.

On paper, its just okay.
It has six resistances, two of which are against two of the most common offensive types (Fighting and Ice) as well as the ubiquitous Water. In exchange, it has five weaknesses, but only one is very common (Electric), though with the popularity of certain Dark moves a Dark weakness isn't helpful.
In terms of offense, its also decent. It only hits 228 Pokemon super-effectively, which is a bit below most type combinations. Only 28 Pokemon resist it though, so its not terrible. Psychic is not a good attacking type, and usually only used with Psychic types who lack coverage.
This is not a problem for Water/Psychic, however, because as Water-types, they get Ice Beam, which rockets their effectiveness to 397 super effective and only 12 resisted.
Additionally Starmie gets Thunderbolt, which combined hits a whopping 501 Pokemon for super effective damage and is not resisted by any Pokemon (the only Pokemon immune to it is Shedinja).
So its pretty decent, but Psychic ultimately does it no favors except for Fighting resistance.

Rock/Ice is a fantastic offensive type. It hits a bit less than half of all Pokemon for super effective damage. It is without a doubt the worst defensively, however.

Not really. Fairy and Ghost both borrow heavily from mythology, especially Japanese mythology. There are numerous yokai-inspired Fairy and Ghost types.
Fairy's opposite type is Fighting, Dragon, or Dark.
A huge array of Pokemon can learn both Shadow Ball and Dazzling Gleam. For example, both Gengar and Sylveon.
