Adding shit on the fly in game would be fucking impossible. Every five minutes the dm would go "yo, wait for a while", use the whatever system to drop shit around the map and wait for the scene to load.
And improvising and adding shit on the spot is literally a whole modus operandi for every gm ever.
If you want a pre generated story mode that would railroad your players to the prepared scenes, you should consider using a fucking gamemaker. Or Neverwinter Nights.
Just play an actual tabletop game, Jesus fuck, you're already sitting in one room. No tabletopfag would actually touch a system like that, and normalfags would rather play Smash or Mario Party.

Because what's the appeal? If you have 4 people together, in person, why not just play the tabletop at that point? The tools will be much more flexible and the game can essentially be free form, as compared to software which has clear limitations.
I mean look at Sword Coast Legends, which touted a real-time GM mechanic as a feature. It came out and nobody gave a fuck because it was so incredibly limited and couldn't possibly hope to live up to any tabletop game in the universe.

Hell, you can even expand on this idea and apply it to other genres. There are definitely many kinds of games where four people play co-op while a fifth arranges the challenge of the game they're about to face.
The main problem I see with games like this is that they absolutely require five people in one room to be any good. Games basically need some kind of single-player function to have any appeal at all outside of isolated cases where you have many people in one room.
