Last Remnant, but the difficulty was mainly just figuring out how the systems work.

None to be honest. Excessive grinding/or lack of it, obscuring systems/mechanics on purpose to the player, starting them weak in an area of much stronger monsters, etc. are all artificial difficulty bullshit and usually the main ways JRPGs tend to be "hard". Even the "gimp yourself by being lower level by not grinding a lot" is artificial as fuck, too in an even lamer way (self-gimping challenges don't offer anything new in most games unless they are designed to have fun with it like low percentage Zero Mission speed runs.)

Level 40 enemies placed on the same maps as level 20's. Enemies that have distance attacks that reach half the map. Regardless of difficulty setting, enemies chain combos at every given chance. You need your entire party to reach the exact same exit in order to leave, yet grouping them massively increases the odds for the enemy to combo.
The game was touted for being the Dark Souls of strategy JRPGs, but the game is programmed with the odds greatly in the AI's favor.

Then I assume you usually only play simple, turn-based games and grind to make them even easier. Xenoblade is not a hard game, even when you're several levels below all the bosses.

One of the Saga games. They're generally just hard as fuck, but most of them do things like punish you for grinding and have mechanics designed purely to fuck you up.
I remember one of them (Romancing Saga was it?). You're told to collect 10 gems throughout the game otherwise the bad guys steal them and that is bad. But you know what? Everytime you collect a gem, the last boss gets stronger. If you collect all 10, the last boss turns into a super fucking boss that has about 1 attack. This attack causes instant game over and the boss is programmed to use it 4 times in a row.
