There are interesting quests in almost every town. It really is mindblowing on your first play.
The sidequests to the story are all great and blend seamlessly into the open world.

I think it's as not empty as a medieval map can get. You can't make it as dense as GTA because that wouldn't fit the scale of it.
I was just playing it for a while, I was wandering randomly in Skellige then come across a shepherd fighting some sirens and he told me where he learnt to fight.
It's interesting you could come across some little things like this that's not marked on the map.

I remember when I had to get from one side of the big Skellige island to the other and as I didn't feel like fast travelling with a boat I ended up riding Roach.
Took me a few hours more than planned because I kept taking Witchers contracts and quests. Good time.

There's plenty of viable criticism for the game, but empty on interesting content is just wrong. For every two or three bandit camps or hidden treasure there's a worthwhile sidequest. Easily could spend 30-50 hours on worthwhile sidequests alone.
