Eh if your tears get OP enough the bosses won't live long enough to be any sort of threat, and even then pretty much every bullet pattern in the game is telegraphed or deterministic and therefore fairly easy to avoid.
For new Isaac players in here be sure to get missinghud2, its an exe which puts a HUD on the screen which displays all your important stats like tears and damage and such, and updates just as quickly as the game does. Doesn't interfere with achievements or unlocks or anything either.

Do the challenges Waka Waka and Family Man to unlock Death's Touch and Epic Fetus, the first one is a big damage up + piercing tears, and the second replaces your tears with guided missiles that do your tears damage*20 to enemies.
Don't be afraid of devil deals, health is pretty easy to maintain and replenish if you learn enemy patterns.
Look out for rocks which have a full X on them (in earlier floors they're also a different colour), if you bomb them they drop a couple of soul hearts or keys/bombs, and rarely a golden chest.
Greed mode is a good way to experiment with items and characters because you get a lot of choice in what to run with, additionally it puts you in quite lengthy fights so you have to learn dodging to progress.

Anti-grav tears with brimstone/azazel are so annoying to use, you have to charge up to place the tear, then it charges up again before it fires. Fast moving enemies and ghosts are too much of a ballache to fight with it.

Either Brains or Guardian.
Brains has you blindfolded and your only weapons are three bob's brains.
Guardian makes you invincible but you have to protect an NPC which wanders about, if it dies then you die. It doesn't sound bad on its own but the thing is so fucking stupid, it wanders into spikes, doesn't dodge in any way, and gets priority targeted over you making a good variety of champion bosses genuinely impossible.
The Keeper is a shit character too if you don't get one his game-breaking infinite money combos, and his unlocks aren't worth his difficult at all.

The moles are difficult to hit without any tear effects. It's really not as tough as you're describing because you have the added benefit of being able to put a beam in the enemy's predicted path.

In rebirth it was beating endgame with Lost, since he didn't start with d4/ghost tears/mantle.
In release afterbirth it was Speed!! challenge. Several rooms had unavoidable enemy layouts because your movespeed was normal, unlike now. The only way to beat it back then was to get a god-tier combo like kamikaze+pyromaniac, or otherwise be able to bomb all the doors open.

The items, rooms, greed mode, and one of the two characters are great.
The changes to existing characters are also really good, like giving Lost Holy Mantle so you get one free fuckup per room and the ability to take curse rooms is a godsend.
More items synergize with each other too, say for example you can take double shot and triple shot, ending up with 5 shots total instead of triple shot overriding double shot.
The problems mainly lie with the Keeper who is a tragically shit character, the challenges at least one of which is more RNG than the rest of the game combined, and the attitude of the developer who withheld content because he was afraid someone would datamine his super secret character.

A run has to be very shitty with a lot of bullshit for me to quit. I almost quit one of the dailies because I had zero damage upgrades and encountered Krampus who proceeded to spam Brimstone lasers until I was down to 3 spirit hearts.
