There was a puzzle like this in Wildstar where you had to figure out a code by referencing the telephone button a buncha letters referred to. I dunno, it seems like a lame puzzle if I have to reference something outside of the game to figure it out.

Oh, I have found memories of this one.
I remenber being in class when I was 13 with a friend in the middle of the lecture playing with his DS when we came to this puzzle. He looked at it for a couple minutes and asked my friend for his cellphone. One of those shitty old phones with a whole keyboard embedded on them. We solved the puzzle there and them.

Final Fantasy X's Cloister of Trials puzzles are all pretty awful, if you can even call them puzzles. They're all just trial & error sequences involving you moving little spheres into different slots. The only dungeon that isn't moving spheres into different slots was stepping on tiles. There's no hints on how to solve the puzzles either. It's just a guessing game with no punishment for getting it wrong.

It's particularly problematic for two reasons.
One is that it particularly expects and requires familiarity with QWERTY keyboards.
Two is that one of the hints is "she has a texting thumb".
The act of texting can mean something else to a lot of people, where text messages are written using a phone's number pad. At the game's time of release I dunno how prevalent smart phones with touch screen QWERTY OSKs would've been.
Some countries do not use QWERTY keyboards either, such as those that use DVORAK.
