They already have an "adult" rating lined up and used in the past and many games feature incredibly gruesome violence and killing and torture. Given the west's conservative nature, no retailer wants to be associated with carrying adult-rated material. Same reason you can't find porn DVD at Walmart. AO games would likely only be sold at adult novelty stores on the outskirts of cities across county lines.

There are a couple of niche vendors that translate Japanese VNs and a few porn games.
Problem is that porn + vidya sounds great in theory, but is terrible in practice (outside of the case of VNs).
Pretty Soldier Wars 2048 makes an attempt at being an SRPG, but is so brutally difficult that you'll just hit the tubes half way through the second mission.
Brave Soul was designed with Window 3.1 in mind and has aged like fine milk outside of the gorgeous CG work.

Not exactly, but the console market is huge potential source of revenue for most devs and Nintendo, Sony, and MS all don't want games that get an AO from the ESRB on their consoles.

Translation was done by RCY Bros. Most VNs of 90's vintage were translated by fly-by-night nobodies. Himeyasoft and most other niche players folded by the early 2000's. Did you play a fanlation or the original release?
Thing about Divi Dead is that once you get past the clunky intro, it turns into a pretty intense thriller. It does go batshit a few times, but that's the story itself, not the translation.
Most other VNs at the time delivered either brainless sex romps or endless pap. Target Pheromone manages to drag on and on without managing to build up anything resembling a story. Nocturnal Illusion focused on sex, but still managed a decent story in between scenes (and an interesting take on some western fairy tale characters).

Western games do sex better than Japanese games. Japan's deal is throwing barbie dolls at the screen and showing them in compromising positions. They don't have any naughty bits modeled and never do until some weeb makes a nude mod. Plus, the otaku would riot if a character were shown having sex, since the whole point of most market anime is escapism and Chie-chan having a more successful life than them would make them feel stressed.
In the west, we get long narratives more like a movie which have sex as a part of the story but not the focus. However, our character actually gets to bang the girls instead of being a clueless self insert. Those same scenes get censored in Japan complete with covering up the boobs.
But in general what others were saying is also true, sex and jerking off is a maybe 5-10 minute experience but nothing you do for hours on end. A whole game based around fucking just doesn't have the returns needed for something good.

Let's take a hypothetical game: Metal Giant Snake : Peeping Walker. Some sort of parody game of MGS where you're going around a female army barracks peeping on soldiers, maybe using the tranquilizer dart to be able to fuck them. Maybe they took the president's daughter hostage, I don't know but you're there. You're talking programmers, artists, voice actors, probably hiring porn stars to make cutscenes, and advertising. The VAs and probably the main character at least will be some famous porn star to sell just a couple more copies, but would have to be paid as well.
Now how do they earn the money back? Already it's a PC release only since consoles won't allow it. It's not going to be on the shelves of any major retailers, so it has to be sold through their online store and the adult store across town. Maybe some specialty shops would have it but you'd end up having an experience like the Senran Kagura commercial.
