If it's a question in the middle of class like in the OP, the teacher literally just covered the topic. And the mid-terms/finals are just more repeats of the topics you've already been asked about.

Only around half the time. While every single test question's answer is given to you in class usually a few weeks before the test, around half of the random questions in class don't give you the answer beforehand and require real Japanese knowledge.
