Miller was never a physical trainer, he was an instructor, like a drill sergeant, he didn't actually engaged in making push ups along with the soldiers, he merely gave the commands and tips (much like his role in PW/GZ/TPP)
Youre thinking that miller was some sort of kung fu trainer, he wasnt.
Big boss trained the soldiers physically and mentally. Miller was just a support.

It's weird because that scene in OP and the rescue scene definitely imply something happened to his eyes, but he never acts like he has sight problems. Also, this PW image shows it's not a stretch that he always looked like that.
His eyes were damaged due to constant exposure to bright lights and sleep deprivation while being tortured by the Soviets. He's not blind, just sensitive to light.
If the whole cataracts = PW is true then that would imply Big Boss might have caused it when they first met.
