I could be wrong, but from my experience breaking Greed mode, Chaos shuffles items around in the pools. So it's not really "every item available" but rather that some items swap places. So you might get brimstone in the shop but proptosis might now only appear in an angel room for example.
I figured breaking greed mode and using chaos would allow me to get every single item in the game but I seem to run into getting breakfast a lot faster using chaos than not.

Most of the newer screenshots are from greed mode. It's easy as shit to break. Just get an IV bag. Ideally you'll also want bloody penny, swallowed penny and/or piggy bank. Enjoy infinite money and items.

Protip: Do NOT use Card Against Humanity on the main room in Greed Mode, shoot all the poop, then leave the room. When you return, the button to begin the battles may not be there, and restarting doesn't help.

Things I would do:
3 Mushrooms: Become a dumb mario facsimile. You shoot bouncing fireball tears
3 injections: holding the shoot button charges an attack and when released Isaac will charge forward
3 holy items: isaac's shot is replaced with an epic fetus style attack that uses light pillars instead of missiles.
3 evil items: Become the black isaac in full (not azazel). Devil deals are free and you get a different ending for beating the game in this form because reasons. I can't think of a good shot modifier though.

Shop becomes Devil, Devil becomes Boss, etc.
This shifts every time you leave.
I figured that out when I bought all the shop items until breakfast when I had chaos, left, and came back and bought breakfast and then Angel room items started to appear.
It doesn't randomize pools at all, just shifts what room grabs from what poll at random.
With restock, that means if you don't get the pool you want, leave the room, and come back and buy the item that is offered to see if you hit the pool you wanted.
(AKA Blue candle, but you want brimstone, You blue blue candle and raw liver appears. You're now in the secret item pool. Leave and come back, and buy liver and The pact appears. You're now in the devil room pool and can now get a chance at brimstone.)
I can promise this is accurate because it's the laziest way to do so, the easiest way to code, and edmund is a fatty fat fat fat who doesn't like to pull effort at all.

Yes, that's why I recommend resetting for it over the Holy Mantle, as you can just get that through D4 rerolling anyway (with hopes that it's not paired with Soy Milk / Tiny Planet.)

All you really need to beat Boss Rush with incredible ease is Brimstone, Technology, or Technology 2 with homing tears (which shouldn't be that difficult to obtain via the D4.) Boss Rush isn't that big of a deal.

For all of my Lost runs, I just used the Holy Mantle. Never did I find a Stop Watch, unfortunately. Prior to Afterbirth, I would have advised Holy Mantle over Dead Cat strongly, since Holy Mantle can save you an infinite number of times, provided you only take damage once per room, as well as letting you tank a hit from a room without teleporting you out, whereas the Dead Cat can only save you nine times and respawns you in the previous room you were in (which can result in uncalled for lost lives, namely in cursed rooms.) However, since the Holy Mantle goes away upon using the D4, Dead Cat is now an almost objectively better option.
