I don't have the game installed, but I remember that the heroes I lost were that giant that held on to the money cart, the shieldbearer that died during the riot I could have avoided and the old giant during the siege at the very end of the game.
No other optional deaths and I got Rook alive by the end, which is cool because I like him way more than the girl.
I'm feeling pretty good.
