He topped off at Lt. Commander before leaving Starfleet to become the Federation ambassador to Qo'nos. He was simply too hot headed to go on to have a full time command of his own, this is an asset when dealing with Klingons though.

its pretty obvious that he has no desire to obtain real power (see the episode where he duels and kills gowron and gets asked to lead the klingon empire - but refuses and hands it to martok instead)

All strafleetians eventually become admirals. Look at a typical starfleet vessel. 90% of the crew are ensigns which means they are on the officer track. On fuckups like the curly cuckoo Irish guy was an enlisted rank (seriously, he must have trisomy or something to not become an ensign when every Tom and Harry could become one). In real life militaries, Admirals are few because of limited money and fleets to command. In a post scarcity society, this is not an issue. Only your aptitude and willingness. Serious, the Transporter Irish must have fucked up real bad somewhere.

O'Brien didn't go to the Academy, so he's not an officer. It's just that simple. And there are only so many officers: the Academy doesn't look to be much bigger than a modern military academy, so only so many officers get made each year. Before Wolf 359, there weren't that many ships in Starfleet (since the 40 ships destroyed there were a huge blow), so even if everyone was an officer in TNG (many of the extras in TOS and STVI were enlisted), there weren't strictly that many officers. And as we see in "Tapestry," cucks don't get promoted at all, let alone to command rank. Most people probably retire at Lt.Cdr.
Also, Star Trek isn't strictly post-scarcity, since resources are still limited. Only Humans (and probably just Earthlings) don't have money; it's not a Federation-wide thing. There's still mining and trading and stuff. Ships take resources and people.
