How hard is it to understand that he believes the lineage aspect of the book should be rather irrelevant in his opinion, BUT, if they do make Jon's parentage important, he would hope that it's not R+L=J since that is incredibly cliche writing and it resolves the Brandon and Ashara mystery that started in book 1
R+L=D would meanwhile come as more of a surprise while having subtle hints that it is real from Dany's forgetful and missing memories, and it would change Dany's character more (knowing she's actually half a Stark, forcing her to rethink her opinions on "the usurper's dogs", she "remembers who she is", a common theme in Dany's chapters, and it makes Rhaegar a much more central character to Dany).
I don't even think Preston believes it's going to happen, he would just prefer it since it would be more interesting writing.

Despite being set in medieval times, medical treatment in Westeros is superb. Jaime would not have been able to survive his maiming in our historic times, but Maester medical knowledge is fantastic.
You just know the Bolton maester is also incredibly skilled since Theon managed to survive his torture. The Dreadfort Maester is probably responsible for stitching and cleaning Theon's finger wounds and stitching and cleaning his groin after his castration so he didn't bleed out and allowing him a hole to urinate out of by cutting into the urethra.
I'm more than certain a Maester helped stitch up and heal Theon's foot. Maybe prescribes him with some weed or root or small doses of milk of the poppy to numb the pain.
