It's a movie about change. Doc is going from person to person trying to find out exactly what is happening all the while consuming large amount of mind altering drugs making us questioning how much is real? In reality this film is not about story, it's about Doc and his relationship with all these different characters. Title, IV refers to the incident where something causes damage to itself. At first this may appear to be about Doc - he's stumbling around trying to solve the case, getting knock out, beat up and put through a lot of nonsense. In reality IV refers to the change from 60's to 70's. In film we see contrast from 60's where everything seems to work, to 70's where problems begin to arise everywhere. Capitalism is presente in character M.W. who is businessman and real estate agent that sells crap and destroys environment. He regrets this morally, fakes he's kidnapping and joins the cult. This is attempt from him to go back in simpler times. The Golden Fang is a big part of this movie, it's constantly broad up but never really answered. It's a boat, it's a group of drug dealers, it's syndicate of dentists.. However what GF represents is capitalism. Dr. Rudy says that the syndicate of dentists was form for tax reasons. This shows that capitalism fuel capitalism. They form a grup that trys to avoid taxes but in doing so they strengthen the heart of capitalism. The drug gang that call himself GF sells heroin to people, which destroys their teeth so they go to dentists syndicate also called GF to get it fix. This shows that there is perpetual loop that if you get sucked in its very hard to get out. As I mentioned earlier this is a movie about change yet the only character who doesn't change is Doc. He is someone who ends he's journey where he began. The first shot of a film is the same sa the last one. He's still living at the same place and doing the same thing. He's life is still the same even thought society has changed.
