Much later Bran was trying to escape from the zombies in the cave and he realized that him and Meera were both going to die because they were not strong enough to open the door at the end of the tunnel and then hold it closed against the undead.
Then Bran remembered his brave friend Ser Wylis the Strong who his father knighted the year Bran was born in recognition of his defense of the North. If Ser Wylis was here he would be able to force open this door then hold the door closed against the zombies while Bran and Meera fled. So Bran travelled back in time and deliberately damaged Ser Wylis's brain, partially lobotomizing him, leaving him retarded but still strong and useful. Turning him into Hodor.
To everyone else Hodor was always Hodor, always had been. The present immediately snapped into place to reflect the change in the past, the past that now always had been this way. When Bran grew up he rode around on the back of Hodor and sometimes possessed him and used him to murder people. And when the time came he mind controlled Hodor to hold that door shut while he was stabbed and bitten and torn to pieces by zombies. Bran had created a reality where he lived by changing the past which changed the present and all he had to do was permanently damage the brain of one man.
As for Hodor, perhaps deep inside he remembered the good man he had been, the hero. And as he died screaming he screamed not only at the pain of the zombies teeth tearing into his flesh, but for the loss of Ser Wylis the Strong, the man whose entire life was not only ruined, but erased from existence.
