I've always liked this theory, but there is many problems with just from a logical stand point:
1. Why would Lyanna "Promise me Ned" and then give Howland one of them? If she was dead then Ned gave him one why would the honorable Ned 'break his promise' and give Howland one?

The main reason I don't think they are twins is due to the show. Something huge like that they'd try to 'plan out more'.
