The battle took place about 50 yards from the wood line. Even though the battle lines met in the open, Stannis must have made his way back to the wood in the midst of battle. Pod and Brienne were stationed close to the battle. She knew he was in the general vicinity. She was probably stalking him the entire battle, waiting for her chance.
How she found Sansa is a little vague. But basically, Brienne finds some allies of Stark a bit earlier, and asks them to send a message to Sansa, who is held captive. The plan with the candle was orchestrated with Brienne. The Stark loyalists were supposed to take Sansa out of Winterfell, and hand her over to Brienne, who will then take her to safety. That's why Brienne was watching the tower the whole time. Waiting for the sign.
As the battle began, Sansa lit a candle candle from the Tower. But only shortly after Brienne left to deal with Stannis. I imagine that after Brienne finished with that business, she must have saw the candle lit. No one delivered Sansa, but dogs were let loose. And that was her clue to give chase.

She was close enough to the castle to not only see the tower window, but also only a few yards away, Pod had a flanking perspective of Stannis's army coming from the wood. So basically, it's totally possible that she noticed the candle shortly after dealing with Stannis. Or perhaps Pod remained on watch, and he noticed the candle. In any case, with the candle lit, Brienne knows to either expect Sansa's to be extracted from the castle, or for herself to go get her.
I know it's a bit of guess work, but with the search party going out, it's not impossible for Brienne to follow the party, if they could lead her to an escaped Sansa.
