They died of rabies, but to the general autopsy it shows up as a cocaine overdose. Coupled with the physiological profile they had of her as someone who attempted suicide several times, it was an obvious decision. It was stated that there is only 3 reported cases of rabies a year, so when he has e people dying who need organs, he's gonna think "oh I should do a test for one of the rarest diseases"

Instead of waiting for the autopsy results before transplanting the organs he just did it. this part is understandable and they may have died if they didn't get a transplant soon. Cox too understood that shit happens and he was going to be fine after j.d. talked to him. the third guy though wasn't in immediate need, could have waited for the autopsy, and just so happened to be a patient cox had gotten close to. this is what actually fucked him up.

The whole point is that those people were about to die. There wasn't any time, if he waited for the test results then they likely would've died before he got the results back. It was either rush the transplant and hope nothing was wrong and they get a chance to live or wait and they die anyways.
