Even in a universe where comic books and superheroes never existed, it can't be difficult to work this shit out. People have figured out far more complicated cases of espionage in real life, and that's without millions of people all obsessed about it.
The demeanor he presents under each guise is enough to do it. Clark isn't as assertive as Superman, probably has a slouch, not the best posture, doesn't have the same mannerisms Superman does. All of those things are important in establishing character. You'd be surprised by how much good posture changes the perspective you might have of a person.
