Also, there was other issues: Gyllenhall's character married into money and Ledger had a wife and kids, which neither men were willing to give up.

They thought about it, mostly Jake's character thought about it, but they weren't willing to commit to it. For them the difference between fucking they guy they really wanted when they could and actually being "out" were worlds apart, and the latter was alien to them.
If you think no one got killed in rural Texas or Wyoming in the 60s and 70s for being gay you don't have a realistic grasp of history and culture at the time, because it seems so different now. There's a very reasonable expectation of at least getting assaulted regularly. But for what it's worth the tire iron scene was something Heath's character was imagining, what actually happened was ambiguous.
