In the comic a panel comes away from the parts holding it, just gets bent inward at the edge, and zombies pour through widening it more. Alexandria becomes totally overrun but everybody is hiding in their houses. Rick and Co decide the only solution is to literally go Rambo and kill every single last zombie. His gf and her kid get stuck and she wont left go so he cuts off her hand.

In the comic, after this event is over, they build a ditch, with spikes, all around the wall. The earth from the ditch they take inside and put it against the wall, making it a rampart to see over the top and also a huge barrier of earth physically behind the wall. The, in front of the ditch all around, they park cars all in a row.
Alexandria becomes impenetrable to zombies. I bet they don't do this in the show.
