I always use my real name for PCs because fuck you.
For random NPCs I have a baby book of names and I just flip through it.
I use misspelt names of obscure historical writers, artists, heroes and villains for plot relevant people. Sometimes it relates to their characters, sometimes it's an ironic name, and sometimes I just liked the sound.

I usually like to say fuck fantasy names.
Something biblical usually works or maybe a fantasy spelling to a real name. Obviously it depends on the character but I like tacking on a little nickname so it would be something like.
Luke Baron, King of Crows.
Although I almost always play Human so I haven't put much thought into names for different races.
