The point of a filename joke isn't to describe what's going on in the file in an obvious and straightforwards way.
That's the equivalent of a joke like "What kind of bank do snowmen keep their money in? A snow bank."
They're already thinking of the punchline, so when you say it, there's no impact.
What makes a joke good is the process of figuring out the joke, whether it's the sudden realization as it's given to them and they are able to connect it back to the set-up, or if it's a process that takes them a few seconds to connect on their own.
That's why obvious jokes are not funny, and why jokes that take a few seconds to get have the most impact. It helps a lot of the initial direction of the file directs them elsewhere, and the filename flips the focus to something equally viable that they were not thinking about initially.

You need to understand how the human brain functions in a crisis.
In that in general, it doesn't.
If you're in a literal life-or-death situation and you have never experienced it, you have no idea how you will act once the shit hits the fan.
Thinking you'd have the presence of mind to even TRY disarm the gunman is the same sort of gung-ho attitude that gets you killed.
When you do experience trauma, your brain shifts into the most primal fight or flight. And even then, its own mechanisms work against it to stop that from happening and you see people turned into gibbering messes, with no higher faculties functioning.
That's why military has to train you through trauma. So you know how your particular brain will be working to make you stupid and can overcome that. It rarely ever happens the first time.

You're giving the "you freeze up in a crisis" WAY too much credence. Most people will freeze up for several seconds, then take SOME sort of action. It'd be one thing if he had several guns or an automatic, and it's unclear if it's safe to try to run or attack him, which could lead to a more permanent freezing, but he clearly shoots 2 people at a leisurely as fuck pace, taking his attention off of most of the people each shot, then takes his time to reload 2 more shots. He does this multiple times.
In fact, none of them, except maybe corner guy, freeze at all. They continuously move, several of them try to beg for their lives even after he kills each person who begs and clearly doesn't give a shit. A few try to run away as slowly as they possibly can. None of them have Fight or Flight on. None of them are frozen, either. This is just a movie where they are meant to die without putting up any sort of fight, so we can see what a badass and/or evil shit the shooter is.

Just like these girls. People do irrational things when faced with a life or death scenario. Most people haven't got a "fight back" instinct any more when facing modern weapons. It takes training to overcome the shock of "this thing might actually kill me".
