That's weird, the one time I considered it I was thinking the same scenario. Mostly because I've actually worked there. You could get up to some real interesting stuff there.

A personal favorite of mine is that John comes to in a church, wearing priestly garb and holding a baby that's definitely not his.

I was thinking of running an alternative version I call "Do it Johnny!" Where the dm is John, and he has to go about his day in a normal fasion, while the voices in his head are constantly trying to trigger him to doing things, instead of one persona completely being on control.

Sitting down for a nice game of "Everyone is Dave" with his friends.

Play it loose and keep it lighthearted.
My last game of EiJ started with John coming to in a dumpster outside a Jew Furry convention, while wearing a dirty, stained, but headless fursuit.
Cue a couple of hours of underwear collecting, killing people with cacti, and attempted presidential assassinations, culminating in the leveling of the first story of a casino thanks to a gas leak John caused.

Continuity can sometimes be wonderful. I ran a first session, and in the second one half the world was ravaged by a plague someone released, then there was a terrorist scare, then John gained superpowers from Satan by abusing disabled people, then eventually the world was destroyed. Twas fun.

Of course, our game didn't end with everyone running out of willpower, ours ended with John dying.

It doesn't matter. If you ask me it's a lot more fun when games don't go on for more than half an hour, so you can pull more scenarios out of the hat.
