Seriously, though, this idea is cool as cold yogurt. It presents a bunch of moral dilemmas right from the get go, and when the players finally come to have to make a decision on the issue it's not gonna catch them out of left field, like goblin babies or something.

It makes people think about killing. Life is precious, and guards and bandits can't simply be replaced out of the ether. Elves would probably be affected the worst.

You know how I'm sure about this? Because if this was for worldbuilding, you'd realize that by the time a reasonably powerful party stumbles into this land, the combination of civil unrest, diving birth rate and shitty "husbandry" would cause irreparable damage to the kingdom.

As a matter of severe irony, the Lich Queen should contact the party herself: If she wanted a nation of undead slaves, she'd have one now. Clearly she needs the ancient X of Y, lost artifact of the Z people to replenish the land, or at least a kickass immigration pitch.

The undead are created from people dying, not children not being born. The only thing you achieve by killing all the living, is that you cut off your own supply of undead.
It would be more logical if she would curse everybody to die at a ripe age of 35, that way they can quickly reproduce and then join the undead legion.

When I did an Undying Empire under an Undying Empress, undeath was uanimous with civil service. If you joined the military or government, you could break yourself in easy as a vampire, and then the idea was that you would in time, gradually and willingly graduate yourself into a wight and if you prove particularly worthy in your centuries of service, you may be Liched by her Majesty herself, the only person in the world to know the ritual (and she keeps it that way).

If didn't enter the civil service, you likely lived a farily charmed life as a normal civilian, just with skeleton servants. Of course, all citizens joined the civil service post death, but civilian skeletons were consigned to manual labour and other low think jobs. The Sun never sets on teh Undying empire, and its servants never sleep.

The word "adult" might be a bit more recent in common usage but the meaning is not.

The Never-dead Queens powers extend only fifty miles at most and experiments are being made to limit this further.

She and her court rule from the Quiet Lands. Her court are made up of those whose keen minds were worthy of preservation in undeath. The best and the brightest and those of great wisdom and sound judgment.

The capital city is not somewhere you can raise a family. Obviously. But in China the majority of people in the Forbidden City were eunuchs so its not that unmanageable.

So long as her curse is limited in its contamination all is well. Or well enough.

This could be a really cool adventuring hook, if you avoid the unfortunate implications of this having been the status quo for some time.

My suggestion would be to make "the blight" less than a year old. Every pregnant woman in the kingdom suffered a miscarriage at around the same time, and no new women have become pregnant since then. A few of the less scrupulous nobles have taken maidens hostage out of fear (under the guise of protecting them, of course), and the townspeople have pooled their resources together to hire a band of adventurers to do what the authorities have thus far failed to do: end the blight.

Naturally, the first thing to do is beat some sense into the local lords' head so that he releases the girls. Then, once the adventurers have made some headway into discovering the Lich Queen's identity and location, she enacts a counter strategy. She offers the women of the realm the opportunity for a cure - but only if they serve. Some women join willingly and others are forced to by their fathers (or local lords), becoming semi-death knights for ten years in order to continue their family lines at the end of their service.

This presents the PCs with a dilemma. The Lich Queen is totes evil, but her strongest minions are just women and girls who want a family. Many of them also have husbands in the army who will refuse to fight when the Lich Queen makes her ultimate power grab (if they haven't defected already). By this point the PCs are the strongest characters in the country, and the only ones who can break into her mighty Fortress of Decay and face her - but how can they possibly do that without killing the very women they're trying to save?

And of course, once she's defeated it's revealed that the phylactery displayed around her neck was a decoy all along, and the PC's next quest will be to travel to her homeland and find where the real one is to stop this from happening again.

If a good GM works with this, you could have a decent plot hook in a somewhat fucked up setting, and the heroes would come in to set things right.
But the thing is with how much OP focused on the sexual apsects of the premise, there's reason to doubt that he would handle this setting properly.

And sadly, there is no "anti-magical realm" setting. Testiclebrained idiots can make anything sexual, just like having sexuality in a setting isn't inherently warping everything into porn.

If you wanted more of an antique, feudal feel, with knights and barons all over the place.

I won't call that magical realm, but I'll call it *slightly* edgy. On the practical side, it does put an impetus on the heroes dealing with the Lich Queen post haste.

Mensus =/= sexual maturity. Given a good enough diet, girls begin to menstrate years before their pelvis finishes widening, before their uteruses finish growing, before their vaginal canals reach their adult size. Sure, it's not impossible for an 11 year old to carry a child to term (the record is 7, Fwiw), but miscarriage rates, mother mortality, and long term health side effects are going to be through the roof unless families manage their daughter's diet and exercise programs with intense scrutiny, In order to keep their body fat abnormally low (like those gymnast girls) to delay the onset of mensus until the girl has grown some (though this strategy will also stunt their final adult size), and then when the matron dietician decides that a particular girl is ready for her mensus, the goal is going to be getting a son and a daughter out of her in only 364 days of possible fertility, so, barring drugs that force multiple ovulation, and assuming that the onset of the curse doesn't abort a second pregnancy, they will throw a feast in her honor, and then pump her with as much sperm as possible over the next 16 days to try and ensure that her first ovulation "takes", they've only really got 3 tries if they want a double.

But even getting 2 kids out of each girl isn't exactly replacement rate, any particular baby is 5% more likely to be a boy, and infant/childhood morality means that the real replacement rate might be 2.4 per woman (modern era) or as high as 5 births per woman (industrial revolution England).

So you won't see harems where one man collects dozens of teenage or preteen girls in a desperate attempt to get heirs, that's population suicide, you'll see ceremonies where each girl gets the sustained attention of dozens of men for over 2 weeks at a pop, during her handful of fertile periods.

And even then, if the gene pool doesn't include a strong tendency towards paternal twins, the society is still doomed (barring modern fertility drugs).

Turns out that it isn't her doing.

The world is simply coming to an end and she is trying to stop it.

By all of her magical resource and effort she managed to sort of buy a bit of time for that kingdom. The fertility rate was abysmally low but its better than her homeland's was.

She turned herself into an abomination to buy herself more time and try and find a cure. All her efforts managed to get that nation one or two more generations.

Beyond that only the dead will live in this world.

Party now has to help her find a cure for a world that is dying or find a way off the world that is dying.
