1) Winter medals should not ever be pooled with the Summer medals for the same index since they have a very different nature and many countries simply can not and do not compete for the Winter gold. Keep them separate.
2) Per capita medals is a considerably better measurement of achievements in sports, but be sure to highlight those 1, 2 and 3 medal small nations that will rank highly
3) Gold medals should weigh much more than Silver which in turn should weigh more than Bronze.

The US is the only nation that takes sports as serious as they do so it's not surprising. The rest of the world sees sports as a pastime and professional athletes as dumb/poor people who have nothing better in their lives other than aiming to become a professional athlete.
