Jack Pardee with the Houston Oilers.
It was 4th down and rather long and they were at the opponent's 30 yard line or so. The Oilers were down by 1 or 2 points. Rather than kicking a field goal, they went for it and were stopped short of a first down.
When asked why didn't kick a field goal, Pardee said that he forgot that it was fourth down.
Marty Mornwheig winning the coin toss for the LOLions in sudden death OT vs Bears and deciding he wanted the wind instead of the ball.

Don't remember the exact game, but UMass in 2014. Down by one, moving down the field with time running out. Get near the goal line with three seconds left, but decide not to spike the ball and kick a field goal, instead run a play, fail to score, and time expires.
