Judging by the feature lists it hasn't changed much from OOTP 16, unless maybe if you like historical simulation.
To me it's always worthy to upgrade it though.

what rating shuld i go with? 20 to 80 seems weird but i don't know if there is an actual reason for it, also do i choose Ratings relative to currently selected league?

I've heard about the sim speed being improved, but I'm a little weary because the 3D gameplay doesn't look as impressive as the developers were touting. But honestly, the auto-facegen is enough of a perk to make me shell out another $40.

im used to the relative to the league option not existing, but since i don't know much about the sport it should come very usefull so i know more or less what im signing and if it's wise to start a young player yet

Sometimes the owner goals are unrealistic/counter-productive. I've been told to offer my "ace" starter a contract extension. The problem was that he was coming off of an injury plagued season with a career worst ERA. Oh, and he was 39...And his player rating had dropped from 4.5 stars (out of 5) to 2.5 stars.
