Filioque means "from the Son", and adding it to a line in the Nicene Creed which originally said that the Holy Spirit proceeded from the Father (and the Father alone) changed the balance of the Holy Trinity.

People took these things very serious back then. There were a ton of other factors, too. The cultural split between the Greeks and the Latins, the struggle between the Pope and the Patriarch, and the fact that the Roman Empire (which was Orthodox) lost all of its power in Western Europe by this point all played a role.

The joke relies on the absurdity of the premise. The only people who seriously argue about those kinds of obscure theological differences in real life are monks with lifelong training. The idea that a professional basketball player would somehow get into an argument about it with his coach and have it be severe enough for them to have a falling out is so ridiculous that it's funny.
You didn't even really have to know what the Nicene Creed or filioque were to get it, just that they were highly obscure theological terms.
