I know this sounds like a set up, but it works.

I've had acne for 10 years now, since high school. I live in thirdworldia and have no access to a competent doctor so I could adapt my diet or whatever.

Using basic soap 2-3 times a day, food is bread/oats/milk/fruit. I have no fucking clue why it still haunts me, 10 damn years. How did people manage it in medieval period? Is air pollution really that impactful? Whatever.

if you have cystic acne its likely hormonal:
+ birth control if grill
+ accutane
+ no dairy

if you have nodular acne or very infrequent cysts:
+ accutane
+ topical retinoids
+ cycline antibiotics (mino, tetra, etc)
+ no dairy

if you have pustules:
+ accutane
+ topical retinoids
+ cyclines
+ benzyl peroxide
+ no dairy

if you have pimples:
+topical retinoids
+benzyl peroxide
+acne.org regimin and all that (wash face only twice, no soaps, etc)

The distinctions arent super rigid. Point is severe acne (cysts, nodules) can't be treated with otc remedies or drugs or "lifehacks" and pretty much 100% require accutane.

Less severe acne you could maybe try some random things to see whats what, but if you have acne thats causing SCARRING, dont fuck around with anything but accutane, because scarring is permanent and lasts for life, no matter what you do.

So if you have acne and its causing scarring, get on accutane.

Also don't go to acne.org, the only people who use that are bitter angry teens whom accutane did not work for, so there is only sensationalistic shit about how bad accutane is.

I listened to those cucks and prolonged treatment with accutane by 1 year, and got about 10 new deep scars. Then i went on accutane, 15g total, 8 months, and the worst i had was dry lips.

You just have to try accutane and see if it works for you, dont listen to hysterical teens and ignorant people who will attribute a red mars to accutane.

yes accutane is so effective because it acts on all 4 causes of acne:
oil glands: reduces size of oil glands semi-permanently
clogged pores: makes sebum less sticky and less prone to clogging
bacteria: makes environment less suitable for p.acnes bacteria
dead cells: increases exfoliation rate, also preventing clogged pores

The 5th "pillar" of acne i would say is inflammation. Accutane, topical retinoids, anti-histamines (hydrocortisone, ibuprofen/advil, etc) all reduce inflammation. Inflammation is what actually causes the scarring on your face, not the acne itself. Inflammation causes collagen to break down in your skin, leading to the pitting we see as scars. The key to managing your acne and scarring is to prevent inflammation: use the things listed above, and also use ice packs, and never pick at the acne or do anything that causes the skin to turn red. This will also reduce the lifetime of the cyst or acne on your skin significantly. Sulfur masks are a lifesaver, and sulfur can also be used for spot treatment against cysts, it works very good.

Anyways, afaik accutane is the only drug that acts on all 4 causes of acne. the others only do 1 or 2 things and that is why for severe acne such as cystic acne or acne that causes scarring, you go straight to accutane.

in terms of strength/effectiveness:
Accutane > Contraceptives > Topical Retinoids = Cycline antibiotics (often combined together) > sulfur = hydrocortisone (both weekly/spot treatments only) > peroxides ~ clyndamicin > tea-tree oil type stuff > 99.9% of herbal/natural remedies.

I waited 10 years of cystic acne before getting accutane the first time and I have so various scars... I did fraxell and it did nothing.

I am doing this currently. I started 9 months ago. I roll a dermaroller 2mm, each month. then I apply some hyaluronic serum for two days. so fr it does nothing.

the roller is nice to destroy a pimple when it comes. I pass a 0.5mm roller, then put a bit of tea tree oil, then I moisturize. twice a day. this is the best method that I have to prevent pimples to settle.

I got burned once, played with fire and it backfired. My shirt caught fire (it was partially drenched with flammable fluid), I was quick enough to take the shirt off to avoid third degree burns bit still got 2nd ones all over my face. My acne scars are gone where I got burned.

hyperpigmentation is just a result of the inflammation caused by the acne, its just an excess of pigments in that area, and takes time to fade.

also i dont think hydroquinone can be used for dark skin, but im not sure. same for laser treatments i think as well.

besides that, topical retinoids (retin-A, differin, tazorac) will reduce hyperpigmentation, but most of all, time is required. it will take a few months if it was a cyst or heavily inflamed, but it will go away eventually.

just be sure to avoid the sun and ALWAYS wear sunscreen, even on cloudy days (clouds dont block the light that causes tanning/pigmentation), because that will make hyperpigmentation much worse and take much longer to fade.

Yeah accutane doesnt work for everyone, it also depends what type of acne you have and what is causing it.

Also the potential side effects suck, but everything has side effects.

In most cases one course of accutane will "cure" you of severe acne, often turning your cysts into regular pimples.

Otherwise if you still have severe acne after the first course, you move on to the second.

It also depends on how much accutane you take, the dose is 100% determinant of the final result.

Effectiveness increases linearly with total dosage (and so does side effects).

Some people are fine with 10g, some need 15g, some need more.

And then there are the unfortunate cases that accutane does not work at all for. Not much can be done about that because there are a lot of factors that cause acne, and accutane only treats the major ones.

shit I'm scared to go on accutane, I have nodular/cystic acne not severe mild-moderate and the only treatment I've tried is epiduo so far no way in hell am I taking minocycline or shit like that because it just kills all the good and bad bacteria in you which worsens your acne after you get off them

it was around 8 months at 80mg/day, a bit of an underdose (~200lb) for a "high" dosage. in total i believe i took around 13-15g.

i had 2 stomach aches, very dry lips, some nosebleeds, and dry skin but cetaphil moisturizer solved that np.

it also dried out my dick (uncirc) seemingly permanently, but its not so bad.

also my forehead seems very sensitive now to moisture/dryness and i think i have a bit of rosacea there: itchy red spots appear and go away within a few hours. i just use hydrocortisone and its fine.

completely worth it though, i havent gotten anything more than little pimples for years now, compared to cystic acne weekly for years.

yeah before accutane i was on the cycline drugs for around 5 years. didn't stop the cysts, and it gave me permanent tinnitus in both ears.

also they can permanently destroy the color of your teeth, fuck the antibiotics imo. accutane is the way to go if you have cysts or nodules.

actually pimples arent even accurate, since they dont come to a head. they're like the most weakest acne ever, just small red spots that go away within a day.

in 3 years i've had a handful of real pimples, no cysts, and one "nodule" (almost a cyst) that went away in 3 days.

i think the nodule appeared because i had stopped using my typical face hygiene habits for about 2-3 weeks prior: daily retinoid (retin-a micro), my aha/bha face wash, and weekly sulfur mask, and washing face morning and night.

that was 6 months ago now and i havent gotten anything notable since. i just thought it had been long enough and maybe my acne was gone for good, but it seems i still need these things to keep it in check, which i am completely fine with (esp since the retin-a heals scarring and skin damage when used for a very long time)

What? No it doesn't. Acne returns after tetracyclines because tetracyclines work by suppressing the inflammation. You're supposed to continue topical treatment (ideally retinoid) after it's done so it returns much less than before.

If you're so worried about your gut flora, take probiotics. Don't deny yourself good treatments because of misconceptions and fear.
