OP is pretty obviously intersex to a degree. It appears from the picture that the flabby lower abdomen could contain an under developed uterus like organ and possibly testes. If op hasn't seen a Dr about this yet I'd be very surprised. If he does they'd probably recommend removal of any malformed organs, trimming the excess skin from the lower abdomen and testosterone supplement.
