Tell me about a time you failed at doing something.

I failed at using a condom when I came inside your mom, and well, here you are
Depends on the type of job. If you're applying to low-level, retail, service jobs you just have to practice with a friend or in the mirror.
If you're looking for a professional job, I'd suggest headhunters. Generally speaking they won't find you a job but you can practice interview skills with them, they will give you feedback and review your resume.
Either way, look up the most asked questions (yeah, the "name one time..." or "what is your best/worst quality?" types of questions and think of an answer and practice it over and over again. This way when the question is asked, you don't freeze without an answer.
