it's a self confidence type of deal but yes charisma, humor and "being yourself" are all you need to score even if you're butt ugly. the trouble is it's just that much more difficult to feel comfortable "being yourself", being outgoing, taking risks making jokes, if you feel like you're ugly or that nobody wants to be around you.
in your fat friend's case he likely had a lot of support through family or friends as he was growing up, or was an only child, or was rich, something that made him feel secure enough that he could be outgoing and take risks making jokes.
