Been on lithium for about a year now. It's effective but not noticeable.
It's a salt which means it's well tolerated, so don't let people tell you it's a harsh drug.
I'm on 450mg twice daily and it helps but I still have a benzo for panic attacks.

Lithium is an extended release mood stabilizer that you take all the time. The idea is to keep a constant blood plasma level so that the drug doesn't contribute to your mood instability the way a daily benzodiazepine prescription would.
So you might experience less fear/paranoia long term but it won't prevent panic attacks. For a panic attack, you can take a benzo that will peak quickly in blood plasma then diminish quickly as the panic attack subsides. You should never take a benzo to treat lomg term feelings like those of fear you're describing.
