So over the last year l've basically created a livestyle for myself that almost completely closed me off from human interaction. I live in a city were l basically know no one and am self employed working from home.
This isolation, while having many advantages made me a complete social retard.
While l was always somewhat dorky, at this point it's just getting ridiculous.
Everytime l outside to buy something l go absolute spagetti when speaking to anyone or even just interacting with a cashier.
How can l fix this?
Find a video game you like, find friends within said video game, start talking to them over teamspeak/skype, gain social skills.
DESU if you play oldschool runescape and join a pvm clan you can make great friends

Desensitize yourself to embarrassment, which is the only thing you have to fear from social interaction. Join a club, pick up girls, go to a bar, do karaoke. You'll make a fool of yourself and pretty soon you'll stop caring too. Once you stop caring it never leaves you, you won't care what people think of you ever again, it's wonderful.

Start playing a few dating sims, you should pick up some social skills from there. Then branch out

pretty much the same thing has happened to me. i spend so much time alone that i can't really talk to people anymore. it's all broken thoughts and i trip over words constantly. it's embarrassing.
apparently this is similar to what prison inmates go through when they spend a lot of time in solitary confinement. isolation just breaks your mind.
