There are two sides to programming a large project.

The first is the ability to understand and create complex algorithms. Finding inverse squares quickly is a great example of this that's mentioned all over this thread. There are other stuff like sorting and pathfinding that you should get good at too. A large part, however, is thinking up algorithms that nobody has ever done before. Being smart helps, but you can actually get better just through practice and experience (contrary to popular belief).

The second part is thinking of how your algorithms should fit together in the grand scheme of your program. Think of a program as a bunch of little puzzle pieces that you have to put together. Honestly, this is the harder part. This is the part that you spend awake thinking about. There is no mathematical way to solve it. Designing software basically becomes more of an art than a science at this point. You can dissect somebody's algorithm and tell them that another way will use half of much memory and process twice as quickly, but you can't really do this with software design. Decisions break down to "well choice A seems more intuitive and flexible and is not that hard to extend, but choice B would be far easier to extend even though it's less intuitive and flexible". Intelligence helps with this part too, but the only way to truly learn is to make several bad decisions and then look back and understand why they were bad.

Depending on the source language, you might be able to use various helping tools. An IDE with good isualisation of the project structure may be invaluable. If that's not applicable, just try working top-down (or bottom-up), i.e. start with the topmost (bottommost) piece of code (e.g. the code that initialises or starts the application) and work your way down (up), exploring the codebase.

I'm currently working on Grade 4 Math on Khan Academy. I never got the fundamentals down and my attention span prevented me catching up when things started getting harder.

I wrote Python and JS before (no experience in Java), and I was hired to write Java and JS. In other words, web software.

The technical interview included an hour of diving into what and how I had written before. I was able to demonstrate in-depth understanding of common design patterns and software engineering processes. That on top of a burning passion for my field and decent social skills sealed the deal.
