Medfag in residency here. This is correct, you don't actually get to apply much knowledge until several years later when you are practicing.
Even then, for most specialties a huge part of the application is learning to hone your intuition and pattern recognition (which you will do by seeing patient-after-patient-after-patient).
Only MD-PhDs and a small subset of specialties (things like Nephrology or Clinical Genetics), actually do any hard thinking. The rest of us just grind.

You get taught the fundamentals from your professor, get to exercise them with the problem sets, but the questions you must solve on the test are unlike any you've ever seen before. It's not like solving a math problem where you follow the recipe of steps until you get to the solution. You have to actively think of a solution which culminates everything you have learned in the past as well as what you've learned in the class itself. Decide which one you think you can realistically handle and not flunk out during (assuming you pass through the worst most depressed state you've entered before, but imagine it happens while in college).
Then try to predict how good the job market for that type of engineer will be, when you graduate. For instance, Food engineering or product engineering might not have much prestige, but if the job market is robust and pay is good, still might be worth it.
Last of all, consider what it is you enjoy learning about, what you enjoy doing, as a personal preference.
