I haven't been to a dentist in more than 35 years. Couldn't afford it. All four of my wisdom teeth became impacted more than 20 years ago and broke into pieces, leaving raw nerves exposed and jagged shards of broken teeth which rip up my tongue and cheek constantly, giving me a never-ending procession of infections and abscesses. I've become so accustomed to never-ending chronic pain that I hardly even notice things most people consider incredibly painful any more, like broken bones.

I've had a few small black spots/lines on a couple of my back teeth (molars?) for over 10 years. They haven't changed in size since I've had them. My teeth don't hurt or anything, but I know it's cavities. At least they haven't grown or spread on my other teeth yet.
My teeth are in fairly decent shape considering I went months without brushing my teeth when I was a kid. Now I brush at least once a day and sometimes floss.

Depends on your clientelle. If you work as a city dentist with relatively wealthy patients you can afford better equipment and higher standards of care, often as good as western ones. But there are many older dentists that work in villages and often dont even do endo because the patients cant afford it, sometimes working as if it was still socialism. It will be a problem when there dentists retire because there is not enough younger ones to replace them, who wants to work in a small village for a relatively shit pay when you can just emigrate to the west? Guess its Ukrainians turn then :)
